If an injury occurs at your work place, follow these easy steps:
- Immediately report your injury to the supervisor or call Sheakley UniComp, Inc. at: 513.618.1249 or 1.888.743.2559.Ìý
- Seek medical treatment from an appropriate network or Bureau certified provider. (All referrals to a specialist, any hospitalization, or diagnostic testing must be preapproved.)
- Actively participate in your treatment plan with your physician to ensure a safe and timely return to work.
All notifications and medical information received by Sheakley UniComp, Inc. will be submitted to the Bureau of Workers' Compensation.
You have the right
- To receive all information pertaining to your injury, and treatment ordered in relation to the injury and how to obtain services.Â
- To be treated in a courteous and considerate manner by all UniComp personnel and providers related to the Sheakley UniComp, Inc. Managed Care Organization (MCO).Â
- To be an informed participant in the treatment of your injury and your case management plan.
- To refuse medical treatment or case management services and to be informed of the effect this may have on your injury outcome.Â
- To review the network physician directory located at your employer site for the names and professional status of the providers caring for your injury.
- To receive prompt medical attention in the case of an emergency situation.
- To confidentiality of information regarding your medical conditions.
- To access the grievance/complaint/appeal process if there is a dispute over medical or non-medical issues.
- To be reasonably informed of all choices in regard to the treatment of your injury.
- To treat with or change physicians to any BWC certified physician.
- To give informed consent for case management services.
- To receive notification and rationale when case management services are changed or terminated.
- To have end of life and advanced care directives honored, if presented.
- To be informed as to alternate approaches when you or your family is not able to participate in your assessment.
You have the responsibility
- To make a full and complete disclosure of your medical history and symptoms before and during the course of treatment.
- To cooperate fully with those providers caring for you.
- To notify your physician and/or employer if you do not understand the nature of your physical condition or treatment rendered.
- To cooperate fully with the case managers assigned to you. These professionals will be actively participating in your care to ensure a safe and timely return to work.
- To be actively involved in the treatment of your injury.
Complete, one-stop medical management
Your Sheakley UniComp, Inc. representative will oversee your case and coordinate your treatment process with you, your employer and your physician.Â
As your personal representative, your case manager will make sure you understand the nature of your injury, the aspects of your treatment and how soon you will be able to return to your regular or alternative duty job.Â
If your medical conditions require you to be away from your regular duties for more
than eight days, your Sheakley UniComp, Inc. representative will be contacting you
to ensure that the treatment you are receiving is both effective and beneficial. Your
satisfaction andÂ
a safe return to work is important to us.Â
